We collect or use your personal details to be able to provide podiatry care. We obtain this information directly from you when you phone, or electronically book an appointment. This information is needed to make sure we can visit you at home, and also to make accurate assessments and diagnoses. We use a secure electronic appointment and digital patient note system called Cliniko. Cliniko is a cloud based clinical software tool ensuring your personal health information is stored securely and only accessible to authorised personnel in compliance with privacy laws and regulations. The information we collect includes your name, address and contact details, your gender and pronoun preferences, date of birth, health information (including medical conditions, allergies, medications and medical history), your GP contact details. We may also use photographs for conditions presenting in the lower limb to monitor progression.
We use this information to create a unique medical record, to contact you about your appointments or about the services we provide. With your permission information contained within your health record is shared with other health professionals.
We suscribe to a clinical system called Cliniko. This is a cloud based practice management system. Belper Podiatry staff have access through passkey or password access. Cliniko are committed to the security and privacy of all data from their clients, and the data recorded by the patients of these registered clients. Only clinicians working for Belper Podiatry have access to your clinical records. We may contact your GP or other health care professional involved in your care regarding your treatment with us. This might be to refer for further treatment or diagnostic tests, to request information regarding assessment or a test you have had elsewhere, to co-ordinate your care with others at your request. This will be discussed with your prior to information being shared.
We do not sell or rent your information to third parties. We may use your information for our own marketing purposes.
We review our retention periods for personal information regularly, We are required to hold some information i.e. your treatment records to fulfil legal obligations. This means that you cannot ask for these to be removed or destroyed. We will hold your personal information on our systems for as long as is statutorily guided for health records.
The statutory period for retention of health records for an adult is 8 years after the patient’s last appointment. For children and young persons under the age of 18, these have to be kept until their 25th birthday. For mentally disordered persons (within the meaning of the Mental Health Act, 1983), for 20 years after the last treatment.
Under UK data protection law, we must have a “lawful basis” for collecting and using your personal information. Our lawful basis for collecting and using this data is because we provide patient care. We gain your consent to use this information after we gave you all the relevant information. Your rights are:
• Your right of access – You have the right to ask us for copies of your personal information. You can request other information such as details about where we get personal information from and who we share personal information with. There are some exemptions which means you may not receive all the information you ask for.
• Your right to rectification – You have the right to ask us to correct or delete personal information you think is inaccurate or incomplete.
• Your right to restriction of processing – You have the right to ask us to limit how we can use your personal information.
• Your right to object to processing – You have the right to object to the processing of your personal data.
• Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you.
• Your right to withdraw consent – When we use consent as our lawful basis you have the right to withdraw your consent at any time.
